(A) His/her educational qualifications as per eligibility criteria (from 10th/ Matriculation onwards) with percentage of marks obtained (please calculate percentage from CGPA / OGPA in advance as per University/Board/Council conversion formula).

(B) His/Her personal details.

(C) Relevant certificates like Caste, Disability, Service certificate, etc., respectively for SC/PH/Government Employee Candidates.

** The step by step process for submitting the application form is given below:

Step-I : Registration of Eligibility Details, Personal Details, and Declaration. Login Id and password will be sent to you through e-mail on registered e-Mail Id.

Step-II : Login to Download Challan. The payment of Application Fee in any branch of the State Bank of Patiala after 24 hours of Challan Generation.

Step-III : Login for Completion of detailed application form after another 48 hours from the time, the payment done in the Bank.

Fee once paid will not be refunded in any case, and the application completed after deposition of fee cannot be withdrawn, neither shall be held reserved for any other recruitment nor selection process in future.
